Why should 2b 3a Blockers like abciximab or eptifibatide produce thrombocytopenia ?
After all they are not designed to kill the platelets.
But in reality serious thrombocytopenia occur in a significant number of patients after reopro
That may demand platelet infusion

There are 2 possible mechanism for thrombocytopenia with these medications.
1. Immune mediated – abciximab is essentially a fab component of antibody. Once bound to gp2b3a of platelet, it get cleared up in spleen as immune complex
2. Pseudo-thrombocytopenia due to clumping of platelets when using EDTA tube especially after abciximab use.
