ASDs are classically oval, but devices are typically circular.(Exact Incidence of ovoid ASD is not known. but it easily exceed 50 % .(Recall, the embryologically ill-fated hole expands around the fossa ovalis not circularis )

Do we have Oval devices ?

Why do we accept this shape mismatch ? What are the potential issues because of that ?

When closing non-circular ASD, the size of device should be considered according to the multiple diameters of the defect. One diameter of the circular device could be too large for the shortest diameter causing a deformity in residual rims. The cardiac erosion after successful implantation as one of the most important complications due to a larger device in the oval defect compared to the circular defect (This text was taken from (Ref 1)

Why the idea ovoid device was never popular ?

Technical issues in sizing the exact oval defects ,device company’s lack of interest. However, designer devices with perfect fit with the help of 3D printing is very much possible.

Is it true ,an Oval device might hold good even in the setting of deficient IVC or posterior rim ?

May be. Some truth in it. I am not an expert to comment.

Do we really require a perfect fit devices ?

When I discussed with an expert , he said there is no need for oval device . A large circular device 2mm larger than the long axis diameter of the oval orifice is not an issue at all . Some how, find it difficult to accept that. What about you readers ? Please respond if you have any comments.

Any study on this issue ?

Yes. one study specifically looked into this. It compared the effect of circular device on oval defects. It concluded there no difference in any adverse outcome , but oversizing is unavoidable with oval defect.. This study , though addressed a vital query ,is never meant to find the truth we want, as it has no oval vs circular device to compare the outcome.

Final message

It is strange ,cardiologists look for perfection and precision in every cardiac intervention,while in case of ASD device closure, size is sacrosanct, but shapes, we are allowed to shrug off. May be things will change.

The following  factors are critical determinants of the success of ASD device closure

  1. Location of the defect ( Only ostium secundum )
  2. Size of the defect (<35mm .Never forget  simple truth , larger the defect shorter would be the  rim )
  3. Shape  of the defect (Please note ,none are strictly circular but most devices are ! )
  4. Eccentricity of the defect (RA aspect of ASD need not match LA aspect)
  5. Length of the rim ( 5mm said to be adequate)
  6. Thickness of the rim ( Least respected parameter .Thin filament like rims are notorious  in sagging the device into RA)
  7. Pre and Per- operative TEE (As Vital as the procedure)
  8. Technical expertise . (This includes extreme patience  of the primary operator .Most sub optimal results and complications are related to this.
  9. Good team ( Not every  interventional  cardiologist should  attempt this !)
  10. Courage to abandon the procedure
  11. Device brand (Probably less important )

Atrial septal defect is one among the commonest congenital heart disease .After years of controversy, there is consensus  now , all significant ASDs  need to be closed ,  at whatever age it is detected.

This rule does not apply to small ASDs without chamber  right atrial and right ventricular dilatation. These defects and PFOs need not be closed .

Over the years , the  controversy  has shifted  from   Should we close ?  to  How to close ?

There are two options available : Device closure , Surgical closure

Final message

Device closure is a complex, costly, often  difficult  and  error prone   cardiac procedure .It needs long term follow up and may  carry a life long risk of major cardiac complication.It is useful only in selected subset of ASD patients. Surgical closure prevails over device closure in most situations.

Is this article  has biased view against this  emerging pediatric  interventional procedure of ASD closure ?

It may appear so . But that is the reality as on 2009 !.May we hope technology evolves further and take our surgeons head on .

2012 update on ASD device closure .

The   hard-ware  as well as the  expertise has   improved a lot and it is on right track to become a real challenge to surgery.

The only issue again is the availability of  rims to mount the device . Another  realistic and sensitive issue  which  have I come across is  , many interventionist cardiologist do feel awkward  when they experience  unexpected rim shortage on table.  They should realise it is not their  fault.

Always be ready to abandon the procedure and refer to the surgeon , according to your  true conscience 

After all , improperly delivered device is  a life long pain for the patient .He has come to you with a  great belief  isn’t !

2014 update

Device closure for most ASDs in both children and adult is  now possible with high degree of success. We have crossed about 50 patient experience. And  I am truly amazed  , how within a short period the device closure is about to conquer the crown from the surgeons ! (Exciting new data are coming from   my colleague Dr Gnanavelu from  the new Super specialty hospital of Government of Tamil Nadu Chennai. )
